As the Stroke Program Coordinator, you will serve as clinical expert liaison among the Departments of Neurology, Medicine, Emergency Medicine and Nursing as it pertains to the care of the acute stroke patients. Further, you will prepare reports and statistical analysis for Quality Assurance, Utilization Review and team conferences. You will act as a resource representative by providing information and facilitating services and assist in the clinical evaluation of patients.
Job Responsibilities Include:
Responds to acute stroke codes. Screen for eligibility for Intra-Venous (IV) or Intra-Arterial (IA) thrombolysis.
Participates in daily rounds for stroke patients.
Maintains New York State Stroke Center Designation and assists with application to become a JCAHO Stroke Center by:
Ensuring all policies are current and reviewed annually
Ensuring all educational requirements are up to date
Conducting monthly Stroke Committee Meetings to review quality measures and implement improvement plans for Plainview and Syosset
Collects data for Plainview and Syosset stroke databases by recording information concurrently and reviewing charts. Maintains both databases. Review data collected as part of ongoing quality assurance program.
Educates ED, Nursing, and Medical staff about stroke protocol. Develops and ensures verification of on-going competency in conjunction with Nursing Education and Medical credentials.
Coordinates with related Hospital and/or Health System services in the continuum of care.
Reports to Hospital-wide Performance Improvement Coordinating Group (PICG) pertinent data on strokes, trends, issues, etc. Participates in System committee and participates in other committees as necessary.
Bachelor's Degree in Nursing, required.
Current license to practice as a Registered Professional Nurse in New York State.
Minimum of four (4) years of clinical nursing experience, required.
Experience with stroke preventions and therapy, preferred.
