Contacts and schedules patient/client's appointments for consultations, meetings with community agencies and/or diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports.
Contacts patients, schedules appointments for consultations, agency meetings and/or diagnostic tests.
Attends and participates in meetings as appropriate.
Sets-up files and maintains records.
Collects and organizes data required for statistical reports and completes reports, as required.
Develops and maintains a good rapport with community and patients/clients.
Provides incoming and in-process orientation.
Based on position, may be required to provide bi-lingual translation and/or guidance.
Performs related duties, as required.
High School Diploma or equivalent, required.
Valid NYS Driver’s license, required.
Minimum of two (2) years experience performing community work.
Based on position, may need to be bi-lingual.
Ability to communicate effectively.
Experience working with the HIV/AIDS population, preferred
